No child may be released from the provider’s address to any person other than his/her parents or other person currently designated in writing by such parent to receive the child. Those people authorized to pick-up the child (including parents) need to present photo identification each day until easily recognized by the provider. The following persons have my permission to pick up my child from the school:
In case of accident, illness or emergency, I authorize Little Sprouts Preschool, its management, employees and representatives to provide emergency medical care to my child, including calling an ambulance and/ or physician for emergency medical treatment. In the event that I, the other parent or the emergency contacts Listed in this form cannot be reached to confirm a course of action, I take full responsibility for the emergency medical treatment required and I agree to pay for any and all costs incurred in such a case. I further agree not to hold Little Sprouts Preschool liable for any consequences arising from such emergency medical treatment.
I/We hereby give Little Sprouts Preschool, its management, employees and representatives permission to take my/our child off the premises and on excursions that will take place during regular childcare hours. I understand that I will be notified of any such trips beforehand, that trips will be supervised and that all precautions will be made for the safety and wellbeing of all the children. I/We also understand that LITTLE SPROUTS DAYCARE AND ACTIVITY CENTRE will not be liable for any accident or injury.
I/We certify that all of the information given on this form is correct and accurate to our best knowledge. I/We promise that I/we will notify the provider, if any or all of the information changes. The school/Daycare has the authority to terminate a child at any point in case the school/Daycare finds the need to do the same. Decision of changing the rules and regulations is completely at the school’s discretion.
1. The fee includes the cost of the school uniform, books, notebooks, activity, cross-curricular material charges. 2. The fee does not include any additional set of the school uniform, summer camp, premium day care. 3. School uniform provided by the school will include: T-shirt, shorts/ skirt and ID card. (Uniform subject to change). In addition to the uniform, parents must arrange for black shoes. An additional set can be purchased directly from the school at the price of Rs.500/-. 4. Registration forms will need to be fully completed before the child is able to commence school. Incomplete forms or missing documentation will NOT be accepted. 5. You must inform us immediately if your child is suffering from any contagious disease/ illness. For the benefit of other children in the school, you must not allow your child to attend school if they have a contagious/disease/illness, as this is easily passed onto other children during normal daily activities. 6. You must keep us informed as to the identity of the persons who will be collecting your school. If someone else will be collecting the child on any occasion, we will require parents to notify us beforehand. Additionally, the individual will be required to present the staff with photo identification for verification purpose. If the school staff is not satisfied that an individual is allowed to collect your child, we will not release them into their care. 7. Please note that it is important for the children and staff that children are collected promptly. Whilst we appreciate that delays are sometimes unavoidable, we do reserve the right to charge Rs.100 (for every 30 minute period) to cover our costs, as staff will have to be paid overtime. 8. Little Sprouts Preschool does not support the exclusion of a child on the grounds of behavior. However, if your child’s behavior is deemed by us to endanger the safety and well-being of the child and / or other children or adults at the school, then it may be necessary to suspend the child whilst we try to address these issues with the parents. 9. The school will not tolerate staff being spoken to in an abusive or threatening manner. Such behavior will result in immediate termination of a school place. 10. We cannot be held responsible for any loss or damage to children’s property. Every reasonable effort will be made ensure children’s belongings are not lost or damaged. 11. Children may be photographed/video'd as part of their development monitoring. I acknowledge that these photographs may also be used for digital marketing, school displays, newsletters, the website and other activities related to the running of the school.
